Hospice Services

SunBridge is dedicated to supporting patients with terminal illnesses, as well as their families and caregivers. With the mission to preserve each patient’s dignity, our hospice partners are focused on alleviating the fear associated with dying through comfort care, pain control and symptom management.

In addition to 24-hour medical care, our centers partner with hospice companies in the community in order to provide patients and families with emotional and spiritual support, along with education and grief counseling. Hospice services for a patient are ordered by a physician based on the prognosis of a terminal illness. Hospice teams may consist of physicians, nurses, home healthcare aides, social workers, chaplains, bereavement support coordinators and volunteers.

Recognizing that each patient may have unique needs, our centers will provide the following levels of hospice care.

  • Inpatient care: short-term care provided when symptoms require additional medical attention that cannot be managed in a residential situation.
  • Respite care: short-term relief from daily care responsibilities offered to the patient’s caregivers.
  • Continuous care: intensive nursing care that may be provided in any setting during a crisis period to achieve management of pain or medical symptoms.
