Our care and rehabilitation centers strive to provide quality care to adults of all ages who need short- or long-term care. Each center offers an array of healthcare services for people who have complex medical conditions or who are recovering from surgery or illness. Patients and residents are cared for by a skilled, multidisciplinary team that consists of doctors, nurses, rehabilitation therapists and support staff.

• Short-Stay Care
  Our short-stay care services are designed for patients who are recovering from surgery, illness or other medical events. Our clinical team members are specially trained to care for orthopedic, cardiac, pulmonary and neurology patients. • Residential Skilled Care
  The family-centered and home-like settings found in our residential skilled care centers help create a feeling of community that benefits residents and brings comfort to their families. • Rehab Recovery Suites®
  Our Rehab Recovery Suite® (RRS) are designed to help patients recover quickly – most in less than four weeks – from orthopedic or reconstructive surgery, trauma or a neurological condition. Our multidisciplinary care team works in concert with each patient’s physician to create a personalized clinical care plan and rehabilitation protocols. • Solana Alzheimer’s Care
  Our Solana program, offered in a select number of locations, provides care for a loved one facing the complex cognitive, emotional and physical effects of Alzheimer’s and related dementia. To help our residents and their families feel at home, we deliver that care in a safe and supportive environment that includes daily, meaningful activities that keep our residents engaged.   SunBridge is dedicated to supporting patients with terminal illnesses, as well as their families and caregivers. • Enhanced Nursing Services (VitalCare)
  The clinical care staff at our SunBridge care and rehabilitation centers consists of a multidisciplinary team of professionals that includes doctors, nurses, therapists and support staff. A select number of our centers have nurse practioners, registered nurses with advanced nursing education, post-graduate degrees and training in the diagnosis and management of complex medical conditions.   For seniors who need a little help in handling the daily tasks of living, our assisted living centers strive to provide a warm, caring community in which individuals can thrive while remaining independent.
