We offer a choice of medical plans such as Preferred Provider Organizations (PPO), Point of Service (POS), Exclusive Provider Organizations (EPO), Limited Medical Plan, Preferred Medical Doctors (PMD) and Health Maintenance Organizations (HMO). The availability of plans is determined by your home zip code. |

We offer two plans with one major difference: one includes orthodontia for dependent children and one does not. |
| Vision Insurance |
Our vision plan is administered through Vision Service Plan (VSP) and offers in-network and non-network coverage for eye exams, lenses, frames and contact lenses. |
| Life Insurance |
The company provides, at no cost to you, basic life and accidental death and dismemberment insurance coverage at 1x your base annual salary. |
| Supplemental Life Insurance |
To supplement the company-paid basic life and AD&D insurance, you can purchase optional life insurance/AD&D at reduced group rates at one to five times your base annual earnings. |
| Dependent Life Insurance |
Dependent life insurance is available for your spouse or domestic partner and children through Met Life ($10,000 for spouse or domestic partner or $5,000 for dependent children). |
| MetLife Critical Illness Insurance |
This plan is a voluntary benefit designed to complement, but not replace your current medical and disability income coverage. MetLife Critical Illness Insurance can help you bridge the financial gap between what your existing insurance covers and the additional expenses associated with a critical illness. |
| Short-term Disability |
This benefit amount is 60 percent of your base pay up to $500 per week until 180 days if you are ill, pregnant or in a non-work related injury and unable to work for more than 14 days (no waiting period for immediate hospitalization or accidents). |
| Long-term Disability |
This plan provides income replacement beyond the first 180 days of disability, and the amount is 60 percent of your base pay up to $15,000 per month. |
| Healthcare Flexible Spending Accounts |
A healthcare flexible spending account allows you to direct part of your pay on a pre-tax basis into a special account that may be used throughout the year to reimburse yourself for certain out-of-pocket healthcare expenses. |

Mass Mutual administers the Sun Healthcare Group 401(k) retirement plan. Employees are eligible to participate after they have completed three months of service. The plan offers a variety of investment options, immediate vesting and optional discretionary employer match paid annually. |
