  • Nursing – Certificate
    • CNA – Certified Nurse Assistant (state-trained)
      • Responsible for providing residents with routine daily and restorative nursing care and services in accordance with each resident’s assessment and care plan. Ensures residents needs are met while maintaining the highest degree of dignity.
      • Completed an approved certified/licensed nursing assistant program and has a current certification/license in state of employment.
    • CMA – Certified/Licensed Medication Aide
      • Assists in the administration of medications and treatments to residents in accordance with state law.
      • Nurses’ aide, state approved, for at least two years.
      • Current nurse aide certification/license.
      • Medication aide license from state-approved training program.
      • Long-term care experience preferred.
    • Nurse Staffing Coordinator
      • Provides and ensures that, on a daily basis, adequate nursing staff are scheduled in numbers that are in accordance with current federal, state and local standards, guidelines, and regulations that govern the long-term care center, and as may be directed by the director of nurses or designee to ensure that the highest degree of quality care is provided at all times.
      • Nursing or healthcare experience preferred.
    • Central Supply Coordinator
      • Assists in providing supplies and equipment in an efficient manner.
      • Nursing or healthcare experience preferred.
    • Restorative Nurse Aide
      • Responsible for providing residents with routine restorative nursing care and services in accordance with each resident’s assessment and care plan and as directed by supervisors.
      • Completion of a restorative nurse aide training program.
      • State-approved nursing assistant.
  • Nursing – Licensed
    • LPN/LVN – Licensed Practical/Vocational Nurse
      • Responsible for assessing and providing direct nursing care to residents and supervising nursing activities performed by nursing assistants in accordance with current federal, state and local standards, guidelines and regulations, to ensure that the highest degree of quality care is maintained at all times.
      • RN degree.
      • Current CPR certification.
      • Current RN license in state of employment.
    • Unit Manager
      • Supervises the daily nursing activities of a designated unit in the center.
      • Member of the center’s clinical management team.
      • Licensed nurse/registered nurse preferred.
      • Medication aide license from state-approved training program.
      • Active RN/LPN/LVN license in state of employment.
    • Assistant Director of Nursing
      • Assists the director of nursing services in planning, organizing, developing and directing the daily functions of the nursing services department in accordance with current federal, state and local standards, guidelines and regulations that govern the center and as may be directed by the administrator, the medical director and/or the director of nursing services, to ensure that the highest degree of quality care is maintained at all times.
      • Licensed nurse/registered nurse preferred.
      • Active RN/LPN/LVN license in state of employment
    • Director of Nursing
      • Plans, organizes and directs the overall operation of the nursing services department in accordance with current federal, state and local standards, guidelines and regulations that govern the center, and as may be directed by the administrator and the medical director, to ensure that the highest degree of quality is maintained at all times.
      • Licensed nurse/registered nurse required.
      • Active RN license in state of employment
    • Customer Care Specialists
      • Performs pre-admission screens and provides nursing center with necessary information about patients in order to facilitate the admission decision and ensure that appropriate quality of care is provided. Obtains necessary information and documentation to ensure that patient is both clinically and financially appropriate for the center.
      • Develop and manage the hospital and physician relationships with the center(s).
      • Markets and promotes assigned center(s) in order to maintain a good reputation in the community.
      • Must be a licensed practical nurse or registered nurse in good standing and currently licensed by the appropriate state.
      • Experience in sales in the long-term healthcare environment preferred.
      • Ability to understand and interpret financial concepts and computations.
      • Positive attitude toward the elderly.
    • Alzheimer’s Program Director
      • Responsible for the development, implementation, management and direction of the Alzheimer’s program.
      • Bachelor’s degree (master’s preferred) in human services (social work, recreation therapy, gerontology, psychology, sociology), nursing, occupational therapy, or other clinical discipline, along with the understanding of social model of care, geriatric care experience and preferably having expertise in Alzheimer’s and related dementias.
      • Two to three years of demonstrated knowledge/experience with dementia-specific activity programs preferred.
    • Staff Development Coordinator
      • Responsible for planning, organizing and directing all center orientation and in-service education in accordance with current federal, state and local standards, guidelines and regulations that govern the center, and as may be directed by the administrator and the director of nursing services, to ensure that employees are trained in order to provide quality person-centered care.
      • Demonstrated experience in adult education and training preferred.
      • Nursing degree (RN degree preferred or when required by state regulations).
      • Current RN or LPN/LVN license in state of employment.
    • Clinical Case Manager/MDS Coordinator
      • Coordinates the development, completion and transmission of the resident assessment in accordance with regulatory requirements; reviews and manages reimbursement in compliance with policies and procedures. Responsible for case manage of residents covered by either Medicare or managed care insurance programs.
      • Registered nursing degree; BSN preferred.
      • Demonstrated proficiency in RAI/PPS processes.
      • Current RN license in state of employment.
    • Restorative Nurse Coordinator
      • Organizes and supervises the provision of restorative functions in accordance with regulatory requirements and policies and procedures.
      • Demonstrated experience in restorative/rehabilitative practices preferred.
      • Licensed nurse/registered nurse degree preferred.
      • RN/LPN/LVN with current state license.
  • Rehab Therapy
    • Rehab Aide
      • Assists the therapists and licensed assistants with patient/resident related activities and direct patient/resident care. The rehabilitation aide may assist with treatments only.
      • Previous experience in patient/resident care in a nursing center, hospital or rehabilitation center. Therapy students considered.
    • Physical Therapist Assistant
      • Provides physical therapy services to residents by assisting in the assessment, treatment, program planning and implementation, related documentation and communication. Functions under supervision of the physical therapist.
      • Associate’s degree in physical therapy from an accredited program; successful completion of National Certification Examination for Registered Physical Therapist Assistants; current state licensure or licensure eligible.
    • Certified Occupational Therapy Assistants
      • Provides occupational therapy services to residents by assisting in the assessment, treatment, program planning and implementation, related documentation and communication. Functions under supervision of the occupational therapist.
      • Associate’s degree in occupational therapy from an accredited program; successful completion of national certification examination for registered occupational therapist assistants.
    • Physical Therapist
      • Provides and directs physical therapy services to patients including assessment, treatment, program planning and implementation, related documentation and communication. Functions under physician’s orders.
      • BS or MS degree in physical therapy from an accredited program; successful completion of National Certification Examination for Registered Physical Therapist; current state licensure or licensure eligible.
      • Current state licensure or licensure eligible.
    • Speech-Language Pathologist
      • Provides and directs speech therapy services to residents, including assessment, treatment, program planning and implementation, related documentation and communication. Functions under physicians’ orders.
      • MS degree in speech-language pathology from an accredited program; current state licensure or licensure eligible.
